Uninsured services & fees
OHIP covers most medical visits, but it does not cover every service. Plan ahead with an optional Annual Plan that covers a range of these services for one yearly payment.
Understanding the fees
What is an uninsured service?
It is a service that OHIP does not pay for. These requests still take your doctor’s and clinic staff’s time, so a separate fee may apply. Common examples include:
Notes & forms
Some work or school notes, insurance forms, and medical certificates requested by a third party.
Renewals without a visit
A request to renew an existing prescription without a medical appointment.
Other services
Driver’s medicals, travel-related services, and certain requests for copies of medical records.

Prescription renewals, notes, and forms can come up unexpectedly. The PS365 Annual Plan gives you 12 months of coverage for listed uninsured services, with one annual payment.

Family coverage includes children aged 19 or younger living at the same address. Each person covered must be a patient of Dr. Stewart.
Some services have limits or are discounted rather than fully covered. Please review the fee guide before enrolling.

*Examples include functional ability forms and short-term sickness/accident forms. Coverage applies to forms listed as included in the fee guide. Complex forms and medico-legal letters have separate terms.
The missed-appointment example applies when at least four hours’ cancellation notice is not given. Telephone no-show fees differ; see the telephone-visit policy below.

A few common questions
Before you decide
Does the Annual Plan cover every service?
No. It covers the services listed in the fee guide, with limits on some services. Others have a reduced fee. It does not replace OHIP or private health insurance. Review the coverage and any service-specific policy before making your choice.
Why choose a plan before I know what I’ll need?
You cannot always predict when you will need a prescription renewal, a note, or a form. An Annual Plan lets you prepare for those requests in advance, with one payment covering the listed services for 12 months. It can make these costs easier to plan for and may save you money as covered requests add up. Consider the coverage in light of your own and your family’s needs.
What if paying a fee is difficult?
Please speak with Dr. Stewart or the clinic about your circumstances. Fees can be considered on compassionate grounds, including whether a reduction, waiver, or payment flexibility is appropriate.
Is the Annual Plan optional, and will my choice affect my care?
Yes, the Annual Plan is optional. You may choose it or pay for uninsured services individually. Your decision does not affect your access to medical care or the quality of care you receive from Dr. Stewart.
